I think your best bet to get the best experience - would be to go to an agency like EPM. your run of the mill AMP is going to provide lackluster service and lackluster experience.

Your best strategy is to go a few times first as a single guy and get HEs. Then tell your masseuse that you want to come in with your GF, and your GF won't mind a HE for you. Make an appointment. When you get the couples' massage the masseuse may still be hesitant at the end because she may think you were not telling the truth about your GF, that you were just trying to push her into it or surprise her, so if it were me I would have the GF get up and come over to you to urge the masseuse on.
Just out of curiosity. Did you just get caught, and thought including her will mitigate the situation or did you proactively think of wanting to involve her? I'm young and none of my previous girlfriends would have been comfortable in any of the situations. From my experience anytime I've ever been honest and open upfront about any situation I've been pleasantly surprised by the result.
I think the age of the gf/wife matters. The younger the more probable she will be agreeable. Today women are indoctrinated in sexual promiscuity at a very young age due to omnipresent sexual material everywhere and all around them. Today's young girls are way more likely to prove their worth by acting provocative, they were raised in the age of easy access to porn and their male peers put constant pressure on them to act it out.

I have easy access to casual conversations with 20 somethings and it is no surprise why marriage and or committed relationships are delayed till late thirties or forties. I think young women today are very split on that. While a lot of girls are hyper sexualized, there's a ton of old school, feministic types that aren't. My ex fiance was very stern on not doing anything but kissing on our first date. She was/is a very sexual person with a large sex drive, but was very strict on no sex on first date. There's also a ton of young guys and girls that have a very low sex drive.
