It depends. Sometimes you can get 4H for 1 HF if they are slow. Sometimes you get an unexpected 4H and don't have enough to tip both the full amount (has happened to me a few times). For the tip, you should tip double the usual amount, or slightly less since they are each doing a little bit less work. But the best rule of thumb is to always go in with only what you are willing to spend and leave your wallet in the car. If you expect a 2H and get a 4H there is nothing they can do. What you have is what you have.
Funny thing happened to me about 10yrs ago in a Hell's Kitchen apartment walking distance from my office. These were the good old backpage days and I go to an apartment for an Indy Asian massage. There are 3 girls on a couch so I pick one and go to a bedroom with a massage table in it. The usual biz then she says "ok to touch"? And I say I want you to use your mouth.
