If I may give my humble opinion, if the spa is korean and they are charging $60, and they know you are not LE, then a HE is part of the service, in which $40 is an expected tip. If the HF is over $60, you can bet that there is more on the menu. If the HF is less than $60, (<40) usually run by Chinese and legit, then a $15 fee is expected for a decent legit massage. If you want it to go offsite or have some roaming, then a greater tip is expected. In many of these legit spas, usually the individual is paid a set fee every day, whether they do 1 massage or 8. Some may be as low as $50/day. At six days a week that's only $300. So you can see why their tips are so important to them, as they keep 100% of their tips. In China, they make 10x less than America. This is why they are happy with a $20 tip, but don't expect more at that price, if she does, it's because she appreciates you in comparison to what she was making in China.
